Health education program for diabetes and hypertension
The goal of the project is to implement and evaluate a community-based program to raise awareness, improve both care and monitoring of Type 2 Diabetes and Hypertension patients
DescriptionAct ministries, in partnership with the Handa Foundation, is in the process of developing a community based health education project for diabetes and hypertension. This builds upon a three year work programme in rural health care in the Battambang province which has resulted in the assessment of the health needs of the communities they visit on a regular basis. The prevalence of Diabetes and Hypertension in the Battambang region in 2014 was determined to be 3.2% and 8.9%, respectively. This project focuses on the importance of patient education as a means to empower patients to become involved in improving their own health. It also involves members of the community as peer educators to provide regular support within villages that do not have easy access to existing health facilities and entails a considerable investment in training once suitable members of the community have been identified so that day to day care can be provided by the community with support from dedicated clinics, also held within the communities, on a regular basis for long-term care.
